Lost Document Affidavit Generator — a practical guide

When a certificate, marksheet, PAN, licence or similar document is lost, issuing authorities ask for a sworn affidavit describing the loss before they issue a duplicate — often alongside a police GD entry/FIR copy and a newspaper notice for some documents.

File a police general-diary entry first where the authority asks for it — boards and RTOs commonly do.
Describe the document precisely: number if known, issuing authority, year.
The indemnity clause is what the authority relies on — do not remove it.
Swear before a notary on stamp paper of your state’s value; verify the value locally.

Do I always need an FIR?

Not always — many authorities accept a GD entry or the affidavit alone for minor documents. The issuing authority’s duplicate-issue checklist is the source of truth.

What if I find the original later?

The affidavit commits you to surrender one of the two to the issuing authority — do that; holding both invites disputes.
